 BLAIR OAKS ELEMENTARY SCHOOL

 

AGES AND STAGES QUESTIONNAIRE

Ages:  1-3

Purpose:  Screen pre-school children in the areas of general development, language, hearing, vision, and physical development.

Participation is voluntary; however, those parents enrolled in the Parents as Teachers program will have their children screened.

Personnel:  P.A.T. Teacher

Dates:  Sept.-May.  Children are screened in the home by the P.A.T. teacher.

 

DEVELOPMENTAL INDICATORS FOR THE ASSESSMENT OF LEARNING (DIAL-4)

Ages:  2 yrs. 6 mons. - 5 yrs.11 mons.

Purpose:  Screen pre-school children and in-coming kindergartners in the areas of motor skills, concepts, language skills, and social-emotional development.

Participation is voluntary; however, those parents enrolled in the Parents as Teachers program will have their children screened.

Personnel:  P.A.T. Teacher and other school personnel

Dates:  Fall (pre-schoolers), March (pre-kindergartners)

 

AIMSWEB

Grades:  K-4

Purposes:  Evaluate individual progress.  Evaluate instruction.  Assist in instructional planning.

Areas:  Reading – Phonemic Awareness, Decoding, Fluency

Personnel:  Classroom Teachers

Date:  Fall, Winter, and Spring (at minimum)

 

GRADE LEVEL MISSOURI ASSESSMENT PROGRAM (MAP)

Grades:  3-4

Purposes:  Evaluate individual progress.  Evaluate instruction.  Assist in guidance planning.

Areas:  Grade 3           Communication Arts, Math

            Grade 4           Communication Arts, Math

Personnel:  Classroom Teachers

Date:  Spring

 

MISSOURI ASSESSMENT PROGRAM - ALTERNATE (MAP-A)

Grades:  3-4

Purpose:  MAP-A is a portfolio-based assessment that measures student performance based on alternate achievement  standards.  It is designated only for students with significant cognitive disabilities who meet grade level and eligibility criteria.

Areas:  Grade 3           Communication Arts, Math

            Grade 4           Communication Arts, Math

Personnel:  Special Services Staff

Dates:  Spring


STANFORD ACHIEVEMENT TEST, TENTH EDITION

Grade:  2

Purposes:  Identify individual and group academic strengths and weaknesses.  Evaluate instruction.

Areas:  Reading, Language, Mathematics, Spelling, Listening, Science, Social Studies

Personnel:  Classroom Teachers

Dates:  Spring
